Appointments
For an appointment call (801) 475-8600. Appointments are available Monday, Tuesday, Thursday and Friday from 8:00am - 5:00pm, and Wednesday from 8:00am to Noon.
Appointments can be made in advance for routine and follow up visits. Follow-up appointments can be made over the phone or as you leave our office.
We make every effort to stay on schedule, although emergencies arise. If we are seriously delayed, we attempt to notify patients beforehand.
Cancellations
We ask that if you are unable to make your appointment to please call to cancel or reschedule at least 24 hours in advance. Doing so will allow our office to make that appointment time available to others. Should you not reschedule and miss your appointment (3) three consecutive times due to a no-show, you will be discharged from the practice.
